O'Hara Graudates can downlaid this form and mail it to the school to the attention of the Counseling Center.
Transcript Requests
Any graduate of Cardinal O'Hara High School or Notre Dame High School may acquire a transcript from this office. Forms are available in the office from 7:30 a.m. - 3:30 p.m. on school days. If you are unable to come into the office, we will need a letter authorizing us to release your transcript to a college or employer. We require the following information: Your name, (Maiden name, if applicable), year of graduation, social security number or birthdate (something that identifies you alone), name and address of the college or employer requesting transcript. There is a $3.00 service fee.

Chester County Intermediate Unit will host a College Fair on Monday October 30, 2006 at the United Sports Training Center , Downingtown
from 7 pm to 9 pm.
